Clean style but eye catching. The words Oregon Pain & Spine must be included. I've added some comments as
Note: Because I've gotten so many great entries based on my original theme I've deleted the sample image and am deleting a couple of design comments here to open up the creative field a little. I'm so impressed with the creative talent you all have I want to see what you come up with.
