There are four site locations of Windsor Essex CHC working together as one to support the health and wellness of the vulnerable population of Windsor Essex. The locations are:
Street Health,
Teen Health,
Sandwich Health and
Chronic Disease

Things to communicate through the design
1. Mission - To support the health and wellness of our vulnerable population in everything we do. Vulnerable are those in need of care and system navigation due to identified risk factors
2. Vision - We will achieve our Mission through health promotion, illness prevention and primary care using an integrated model in collaboration with community partnership
3. Values - Compassion, Respect, Equity, Access, Multidisciplinary Care, Community focused partnerships, confidentiality/privacy, accountability
The target audience
Anyone who sees our logo should see that we are here to support the vulnerable population: Children, youth, adults and seniors that are in need of a safe place to receive healthcare and system navigation due to identified risk factors including:
*Social isolation/exclusion (housebound, mental health or substance abuse
*Abuse or risk of abuse
*Economically disadvantaged
*Housing (homeless or near homelessness)
*Mental/physical/emotional health challenges (short or long term/chronic conditions)
* No OHIP or Federal Health Plan
